You also have the option of navigating to the website of the United States Department of State to complete the DS-82 online. Also, you may pick up the DS-82 from any passport acceptance and processing facility, such as you local U.S. Post Office.
Once you've filled out the DS-82, check it and then check it again to be sure you didn't miss anything. Any little mistake, smudged line or illegible entry will mean automatic rejection. If you're confident it's ready to go, be sure to make a copy for your records. Also, sign and date it on the first page where it indicates "applicant's signature".

Even though your current passport will be or is expired, make a copy for yourself just in case something goes wrong. Send everything in a durable envelope using a traceable postal method such as certified mail.
Expect to wait at least 4 to 6 weeks for your new passport. You can request and pay extra for expedited service, which reduces the time to 3 to 4 weeks. Either way, add a few weeks for worst-case scenarios so you won't be stuck with an airline ticket you can't use because you're still waiting for your new passport.
