Along with your application, you will need to show your passport, which must contain a blank visa page. Make sure that your passport is valid for 1 month after the end of your intended stay in the Ukraine. A recent passport photo is also required along with your application.

The application fee is $100 for U.S. citizens. For all other nationalities, the fee range is $45 to $90, depending on how quickly you need the visa. Only money orders are accepted for payment.
